Before we begin any package of care, we visit you to do an assessment and find out what outcomes you would like to achieve. We then use this information to build a detailed Care/Support Plan, which includes risk assessments as well as all the tasks and activities required to help you achieve your goals and outcomes. We use the latest technology to make sure that your needs and requirements can be updated and shared easily and can give access to your loved ones, so they can see that your care has taken place and have a summary of how things went.
